New Apprentice eligibility – Contact the Fund’s office once you have 750 hours or more anytime during the year.
New Change of Address – Contact the Fund’s office for a change of address form, updated Email Address and Telephone Number
New Dependent due to marriage or newborn – Contact the Fund’s office to enroll new dependents, submit copy of birth certificate, copy of Social Security Card. In the event of a new spouse please submit copy of marriage certificate, copy of social security card, and valid Federal or State Identification.
Vision Care Vouchers – Contact the Funds office
Vision benefits are limited to one per year for active members and one every two years for Retiree members.
